The CHHUNY Network of Care Management Agencies frequently changes as new counties and services are being added. For the most recent list of providers, use the search engine below to find a CHHUNY Network Care Management Agency in your area. Many CHHUNY CMAs serve multiple counties and their administrative offices may be located outside of the selected county of service. There are two ways to search:
1. View agencies by counties served by selecting it from the "Choose a County" drop down menu. You may leave the "Search by Provider Name" blank if you don't know the provider's name.
OR
2. If you know a provider's name, enter part or all of it into the "Search by Provider Name" text box.
While these agencies may serve these counties, capacity may be limited.
