Who we are
The Children’s Health Homes of Upstate New York, LLC (CHHUNY) is a newly formed entity established to plan for and operate as a Health Home for Children. CHHUNY is using a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of their care. This model also ensures that families have ‘‘voice and choice,’’ at every stage in the care management process.
CHHUNY was founded and is guided today by several leading children’s health and human services agencies and now includes a network of more than 100 child and family agencies across 54 of 61 counties in New York State. To assure that the CHHUNY is meeting the unique needs of the children, families, care management agencies and network partners; CHHUNY has created a unique management structure to gain the economies of scale from using one infrastructure across a large geographic region while assuring the voice of child serving service providers. Each CHHUNY Board member will function as a Regional Provider Lead for their area of Upstate New York. In this role the Board member will provide a voice and a face for the CHHUNY System of Care Region that they represent. They will manage the provider network for the region by reaching out to organizations (both care management agencies as well as other children and youth serving organizations) in the region. In addition, the lead will remain mindful of the regional system of care culture and offer guidance to CHHUNY based upon those SOC elements that may be unique to their region as well as assure local and regional people and organizations that they will not be marginalized.
CHHUNY will also implement the appropriate board advisory councils to assure timely and effective communication among CHHUNY stakeholders. Council membership will be developed based upon the established expectations.
