In order to receive a contract with the Children's Health Home of Upstate New York (CHHUNY) to provide Care Management services, an agency must complete and submit a complete Business Associate Agreement (BAA) and provide Single Point of Contact (SPOC) information. To assist agencies in confirming that CHHUNY has all of the necessary information to issue a contract, please use the below information:

What to do if your agency is missing information
If your agency has not signed a Business Associate Agreement can do so by downloading the BAA form below. Please write your agency name on page 1 (“Business Associate”) then read, date, and sign as the SUBCONTRACTOR (page 3 & 4) on the attached BAA. Return an electronic copy to Kathy Berthod at Health Homes of Upstate New York (email@example.com) as soon as possible.
If our agency has not provided the Single Point of Contact (SPOC) information, please do so by clicking on the link below and completing the brief template. You will need your agency name, agency MMIS ID (if you are in the process of applying for one please leave this blank), to answer (Y/N) about whether you are a NYS Voluntary Foster Care Agency, the SPOC First and Last name, as well as your SPOC email, phone number, and address. Once you have completed this please email Linda Spas at Health Homes of Upstate New York (firstname.lastname@example.org)

Care Management Agency Financial Support Tool
The excel tool enables Care Management Agencies to project Health Home Outreach and Enrollment, in order to model annual revenues and costs based on the DOH program model (acuity levels and premiums) and a range of user defined variables. The Model allows for the user to modify DOH expectations for Caseload as well as Costs. The Model allows you to build a Health Home care management program (including HH Care Management and HH Assessment activity while including HH administrative fees.
