    • Trauma affects a member’s sense of self, their sense of others, and their beliefs about the world. Trauma-Informed Care means that care managers recognize the presence of trauma symptoms and acknowledge the role trauma may play
      in a member’s life.

    • With CHHUNY’s approach, care managers collect data that will help stakeholders to determine the success of the Children's Health Home program and create meaningful strategies to improve services and health outcomes across New York State.

    • Care managers look to connect the member to resources outside of their typical services that nurture a support network throughout their communities. Children thrive when they are able to make connection and develop relationships within their community.

    • Care managers work to build a multidisciplinary team of providers for each member, in order to best coordinate services. Through a team-based approach, all stakeholders are united in a common goal to maximize effort and impact.
